At Mind Matters, LLC, we are committed to providing essential services regardless of a patient’s ability to pay. We offer discounts based on family size and annual income to help make services more affordable.
Please complete this form to determine if you or members of your family are eligible for a discount.
Important Information
Services Covered: The discount applies to all services provided at this clinic except for DUI or Court-Ordered Substance Use Disorder (SUD) Evaluations, unless special arrangements are made directly through your lawyer, public defender, or parole officer.
Renewal: This form must be updated every 12 months or if your financial situation changes.
Documentation Required: Proof of income is requested to determine eligibility. You may submit & Upload prior to your appointment
Proof of Income – What to Provide
To see if you qualify for our Sliding Fee Discount, we need to know your total household income. Please share documents that show all sources of income for your household, such as:
Job Income – Recent pay stubs, self-employment or business income records, or tax returns
Unemployment or Workers’ Compensation – Benefit statements
Social Security or SSI – Award letters or benefit statements
Veterans or Survivors Benefits – VA benefit statements
Pension or Retirement Income – Pension or retirement account statements
Other Income – Interest, dividends, rental income, trust or estate income, royalties
Alimony or Child Support – Court orders or payment records
Financial Help from Others – Proof of assistance from someone outside your household
Any Other Income not listed above
